I confirm that I am voluntarily receiving this tattoo and am not noticeably impaired by alcohol, recreational drugs, or another intoxicating substance. I understand the artist may refuse or postpone the procedure if I appear unable to provide informed consent or if tattooing would present an unreasonable health or safety concern.
Before tattooing begins, I have personally inspected and approved the tattoo design, spelling/lettering/numbers/dates, size, placement, and direction/orientation shown in the preview I configured. Appearance may vary between artwork, stencil, fresh tattoo, and fully healed tattoo due to skin, anatomy, healing, pigment behavior, aging, sun exposure, and other factors.
I voluntarily request and consent to the tattoo procedure. I understand tattooing involves repeatedly penetrating the skin and permanently depositing pigment beneath its surface, with inherent risks including pain, tenderness, swelling, redness, bruising, bleeding, itching, peeling, scabbing, temporary irritation, infection, allergic or adverse reactions, scarring or abnormal scar formation, pigment loss or uneven retention, fading, spreading or migration, delayed or abnormal healing, the possible need for touch-up work, and other unforeseen reactions or complications. No specific healed result can be guaranteed.
I understand that a tattoo should be considered permanent. Tattoo removal or alteration may be expensive, painful, incomplete, and may result in scarring, discoloration, or other permanent changes to my skin. I nevertheless voluntarily choose to receive this tattoo.
I acknowledge I will receive written and verbal aftercare instructions covering proper cleaning and care, expected healing process, signs of infection or allergic reaction, when to consult a physician, activities/exposures that may interfere with healing, the permanent nature of the tattoo, and possible side effects. Once I leave the establishment, following these instructions is my responsibility.
I knowingly and voluntarily accept the ordinary and inherent risks of tattooing. To the fullest extent permitted by Colorado law, I agree the artist and establishment will not be responsible for injury, complications, infection, allergic reactions, undesirable healing, pigment loss, or other adverse results caused by an inherent risk of tattooing; a medical condition, medication, allergy, or other information I failed to disclose; my failure to follow aftercare instructions; my actions after leaving the establishment; or circumstances reasonably outside the artist's or establishment's control. Nothing here releases or waives liability that cannot legally be released or waived under Colorado law.
I understand tattoos may require touch-up work after healing. Unless otherwise agreed, signing this form does not guarantee complimentary touch-ups. Touch-up eligibility, scheduling, and cost are subject to the studio's applicable policies.
